A regular update for professionals who are using the Notts NHS App, Personal Health and Care Record with PKB.
Long Term Condition Templates on PKB
To help practices and patients increase the use of PKB, we have developed some new Long Term Condition (LTC) questionnaires for patients to complete in preparation for annual reviews for example. These templates have been developed by clinicians in Nottinghamshire and meet the all the necessary practice requirements (e.g. NICE guidance, QOF). This will allow practices to reduce the need for some follow-up appointments and it will also be more convenient for patients.
These templates have been rolled out to a number of practices in Nottinghamshire and we are now looking to roll this out further. The templates that have been created for practices so far are:

We will then look to setup a PKB refresher training session for you, where we can discuss how these templates can be used through PKB (or the NHS App) and work with you to understand how this can fit in with your internal practice processes.
